Intuitive Health Services is seeking a dedicated, highly skilled, and compassionate Licensed Clinical Social Worker (LCSW) to join our healthcare team in Stockton, CA. In this full-time role, you will provide essential mental health assessments, crisis intervention, and clinical therapy to individuals and families navigating complex healthcare challenges. You will act as a pivotal link between patient care, medical teams, and community resources to ensure holistic wellness.
Conduct comprehensive clinical psychosocial assessments to determine patient needs.
Develop, implement, and monitor individualized, evidence-based treatment plans.
Provide individual, group, and family psychotherapy or crisis intervention.
Collaborate with a multidisciplinary healthcare team to optimize patient outcomes.
Maintain precise, timely electronic health records (EHR) compliant with clinical standards.
Assist with safe discharge planning and connect patients with vital community resources.
Education: Minimum of a Bachelor’s degree in Social Work or a related healthcare field.
Licensure: Valid and current LCSW license in the state of California.
Strong clinical, communication, and crisis-management skills.
