About the Role: Intuitive Health Services is seeking a compassionate and dedicated full-time Licensed Clinical Social Worker (LCSW) to join our team in Vacaville, CA. In this role, you will provide comprehensive psychosocial assessments, crisis intervention, and therapeutic counseling services to patients, helping them navigate complex health and emotional challenges.
Key Responsibilities:
Conduct thorough psychosocial evaluations to identify patient needs, strengths, and support systems.
Develop and execute individualized intervention plans, coordinating care closely with multidisciplinary medical teams.
Provide evidence-based individual or group therapy, behavioral interventions, and supportive counseling.
Assist individuals and families with crisis management, advocacy, and navigating community assistance programs.
Maintain comprehensive, compliant, and timely electronic clinical documentation of all patient encounters.
Qualifications:
Bachelor Degree in Social Work (MSW) from an accredited institution.
Active, unrestricted LCSW license to practice in the state of California.
Excellent clinical assessment, communication, and interpersonal skills.
