We are interested in hiring a new case manager to join our team and take ownership of a number of new and existing cases. You should have previous experience as a case manager owning a large portfolio of cases. You will need a bachelor's degree in a relevant field and experience working with clients to develop treatment plans.
Job Responsibilities
- Perform culturally sensitive assessments to identify medical, social, and economic barriers.
- Connect patients with community resources such as housing assistance, nutrition programs, and mental health services.
- Translate and interpret health information for non‑English speaking patients, ensuring comprehension and compliance.
- Lead health education workshops and outreach events in underserved neighborhoods.
- Advocate for patients during multidisciplinary case conferences, ensuring their voices are heard and needs addressed.
Requirements
- Bilingual proficiency (Spanish/English preferred; additional languages a plus).
- Experience working with underserved or minority populations in a healthcare or social services setting.
- Degree in Public Health, Social Work, Community Health, or a related field.
- Strong interpersonal, advocacy, and cultural competency skills.
- Demonstrated commitment to diversity, equity, and inclusion initiatives.
