Key Responsibilities

• Conduct comprehensive clinical psychosocial assessments to determine patient needs.

• Develop, implement, and monitor individualized, evidence-based treatment plans.

• Provide individual, group, and family psychotherapy or crisis intervention.

• Collaborate with a multidisciplinary healthcare team to optimize patient outcomes.

• Maintain precise, timely electronic health records (EHR) compliant with clinical standards.

• Assist with safe discharge planning and connect patients with vital community resources.

Qualifications

• Education: Minimum of a Bachelor’s degree in Social Work or a related healthcare field.

• Licensure: Valid and current LCSW license in the state of California.

• Strong clinical, communication, and crisis-management skills.
