Medical Chart Reviewer

We are seeking a detail-oriented and organized Medical Chart Reviewer to review, analyze, and maintain patient medical records remotely. The successful candidate will examine medical charts for completeness, accuracy, consistency, and proper documentation while ensuring that all information is handled confidentially and according to applicable healthcare privacy requirements.

Responsibilities

• Review patient medical charts, clinical notes, laboratory reports, diagnoses, medications, and other healthcare documentation.

• Verify that medical records are complete, accurate, and properly documented.

• Identify missing, inconsistent, or potentially inaccurate information for appropriate follow-up.

• Organize and maintain electronic medical records and documentation.

• Review records according to established company and healthcare guidelines.

• Assist with medical documentation audits and quality-assurance activities.

• Maintain strict confidentiality when handling protected health information.

• Accurately enter and update information in electronic health record (EHR) systems.

• Communicate professionally with authorized healthcare and administrative personnel when clarification is required.

• Meet assigned productivity, accuracy, and turnaround-time requirements.

• Perform other chart-review and medical documentation duties as assigned.
