Physical Therapist

Intuitive Health Services is seeking a Physical Therapist for a contract healthcare position in San Luis Obispo, CA 93409. Key Responsibilities:

• Assess patients’ physical abilities, movement limitations, and therapy needs
• Create individualized treatment plans based on patient condition and goals
• Provide therapeutic exercises, mobility training, and rehabilitation support
• Monitor patient progress and adjust treatment plans when needed
• Educate patients on exercises, safety techniques, and recovery practices
• Work closely with physicians, nurses, and healthcare team members
• Maintain accurate documentation of evaluations, treatments, and progress notes
• Support patients in improving independence, movement, and quality of life
• Follow facility policies, safety standards, and confidentiality guidelines

Qualifications:

• Bachelor-level education listed for this position
• Active Physical Therapist license required
• Experience in healthcare, rehabilitation, or clinical therapy preferred
• Strong knowledge of physical therapy techniques and patient care planning
• Excellent communication, observation, and documentation skills
• Ability to work professionally with diverse patient populations
